Adults and adolescents (≥12 years of age) with hemophilia A who have been prescribed Jivi^® for an authorized indication are eligible to enroll in the KoKonnect^™ Patient Support Program and the KoKonnect^™ Home Infusion Supplies Patient Support Program.

• The KoKonnect^™ Patient Support Program provides nursing support and training services to assist you with your Jivi^® infusions.
• The KoKonnect^™ Home Infusion Supplies Patient Support Program provides infusion supplies (alcohol swabs, dry pads, bandages) that are required for your Jivi^® infusions.

Ask your hemophilia treatment center healthcare professionals if you require access to the KoKonnect^™ Patient Support Program and/or the KoKonnect™ Home Infusion Supplies Patient Support Program.
